NATIONAL SOCIETY FOR HISTOTECHNOLOGY AWARDS
NOMINATIONS / APPLICATION FORM
The following must be submitted to the Awards Committee:
- An up-to-date curriculum vitae or background of the nominee, showing dedication and
service to NSH.
- Written evaluations attesting to service and dedication to NSH. These evaluations may
come from members and/or current or past officers of the Society.
The Awards Committee will consider your application for any educational/scholarship
award for which you are eligible. Please read
criteria
for Awards. All required and/or pertinent information must be submitted
with this form in order for the Awards Committee to consider.
Please go to the NSH
Award Application page and select only ONE award to indicate your
preference. Students may only apply for student scholarships. APPLICATIONS
MUST BE POSTMARKED BY MAY 1, 2005.
